Mining Financial Assurance Forms

To ensure that mine operators are meeting both County of San Bernardino and State of California requirements, all financial assurance forms must utilize the State’s forms (ref. California Code of Regulations §3504(b) and §3800 et seq.).  Failure to utilize these forms will result in the rejection or delay of approval of your Financial Assurance.  The following forms are provided here for your convenience: 

Certificate of Deposit   

Note: Certificates of Deposit and subsequent amendments must include the issuing financial institution’s letterhead for acceptance.

Letter of Credit

Note: Letters of Credit and subsequent amendments must include the issuing financial institution’s letterhead for acceptance.

Surety – Corporation

Surety – Partnership
